This is linked at the bottom of this article or in the top right hand corner of your screen. Please select 'Wholesale" from the request type drop down menu.
To complete the form, you will need a couple of images to help us get to the bottom of the matter and resolve the issue asap! It can help to have these ready befor filling in the form. Please see below:
Image 1 - Please provide an image of a garments that were received. Please make sure all styles are identifiable and visible and all sizing printed on the garments is show.
Image 2 - If you have it; please provide an image of the packing slip (the white rectangular sticker inside the parcel with your order details)
